A diagnosis of serious illness too often means pain and discomfort, with a loss of dignity, privacy, and independence. Caregivers and family members suffer, too, with anxiety, caregiving tasks, and financial burdens. These can burn up energy and resources in a hurry. As a serious illness unfolds, everything can quickly seem out of control. Yet, it doesn't have to be that way. Hospice can help achieve the best quality of life for each patient and family member. Hospice is not a place; it is a well-managed system of care. Fremont Regional Hospice provides compassionate care with our coordinated team of physicians, nurses, certified nurses aides, social workers, bereavement support, spiritual counselors, and trained volunteers. When medical intervention can no longer offer a cure, Fremont Regional Hospice offers palliative (comfort) care with an emphasis on enhancing quality of life. The primary focus is to provide care in the person's home or extended care facility. Hospice care affirms life and neither hastens nor postpones death. Our experienced staff is dedicated to providing individualized palliative care with services specially tailored to the needs of patients and their families who are dealing with a life limiting illness.

What is Hospice?
As a person's life draws to a close, the time comes when the focus changes from working toward a cure to loving care and comfort.
Who Qualifies for Hospice Care?
If the patient has a limited life expectancy and agrees to the hospice concept of care, he or she is eligible to receive hospice services.
Our Hospice team works closely with the patient's physician. Once the physician authorizes care, hospice services can begin. Referrals can also be made by family members, friends, clergy or health professionals. Services are available to patients without regard for disease entity, race, color, religion, ancestry, national origin, age, sex, sexual orientation, and mental or physical challenges. Who Pays for Hospice Services?
Both Medicare and Medicaid pay for care received form a licensed provider. Some private insurers also provide a hospice care benefit. Although a majority of basic hospice care services are covered by Medicare, Medicaid or private insurers, reimbursement does not always match the true cost of providing care.
As a not-for-profit community based organization, we are committed to providing excellent care for patients with life-limiting illnesses regardless of their ability to pay. Does Hospice Accept Donations?
An individual or organization can donate a tax deductible gift, equipment or supplies. Donations are then used for patient care and helps make our services possible to the community.

Five Wishes is a booklet with easy to use forms for expressing your wishes about how you want to be treated if you become seriously ill. It is valid in 42 states, including Colorado, and can be completed by anyone who is 18 years old or older.

In their role as case managers, they monitor each patient, manage pain and symptom control, and provide professional, compassionate care for your loved one.Certified Nursing Assistants (CNA's)Our CNA's provide personalized physical care, such as bathing and dressing your loved one, under the supervision and direction of the RN's.VolunteersVolunteers are an integral part of our team, providing companionship, support, relief and assistance to caregivers and patients. Fremont Regional Hospice is your only independent, locally owned and operated, non-profit Hospice serving Fremont, Custer, and Park counties in south central Colorado.
We were established in June 2006 and have already helped hundreds of the terminally ill, caregivers, family members, and the grieving patients and families. Fremont Regional Hospice welcomes the opportunity to serve more families as our community becomes familiar with our establishment and the services we offer in the care of the terminally ill.
